Tier 3 and Tier 4 Frequently Asked Questions

The ADI Fellowship represents the highest level of professional recognition within the ADI Fellowship pathway. These FAQs provide answers to some of the most common questions about the Tier 3 and Tier 4 requirements, examinations, eligibility and application process.

The requirements for Fellowship are clearly defined and must be met in full. We recommend reviewing the information below carefully before making an application or contacting the ADI team with a query.

Tier 3: Eligibility & Requirements

I completed a Diploma course but not the case submission—am I eligible for Tier 3?

A: You must meet one of the Tier 3 routes. If a formal qualification was not fully completed, it may not meet Route 1. You should review alternative routes (e.g. specialist status, MImpDent, or equivalent experience).

 

What are the routes to qualify for Tier 3?

A: You must meet one of the following:

  • Postgraduate Diploma/Degree in Implant Dentistry
  • MImpDent (RCS Edinburgh)
  • Recognised mentoring/fellowship pathways (e.g. ITI, CGDent)
  • Relevant registered specialist (e.g. Oral Surgery, Periodontics, etc.)
  • Experienced practitioner pre-2005 guidelines
 

Can I use Tier 3 as a post-nominal?

A: Yes, Tier 3 post-nominals can be used, “Assoc Fellow ADI”.

 

How strict are the Tier 3 and Tier 4 requirements?

A: The rules are very strict, and there are no exceptions.

 

 

Tier 4 Eligibility & Requirements

I have been accepted for Tier 3. How do I apply for Tier 4, and do I need publications?

A: There are no exceptions. You must have the required publications in place before applying for Tier 4 Fellowship.

 

What documents are required before sitting the Tier 4 exam?

A: You must provide evidence of:

  • 2 publications in peer-reviewed journals
  • 2 audit projects
  • Completion of the ADI Fellowship Exam (exam fee: £1,200)
 

I have multiple publications and presentations. Will these count?

A: Publications in peer-reviewed journals are required. Additional posters and publications may support your application but do not replace the core requirements.

 

Are there minimum case requirements for Tier 4?

A: Yes. You should demonstrate a broad range of cases, typically:

  • ~20 bone graft cases
  • ~20 single implant cases
  • ~20 bridge cases
  • ~20 full arch cases
 

What is the required logbook format for Tier 4?

A: The logbook must include:

  • Date of birth
  • Procedure
  • Number of implants
  • Grafting (hard/soft tissue)
  • Technician present
  • Nurse present

You must also provide receipts from implant companies confirming implant usage.

 

My ADI membership has lapsed. Can I still apply?

A: Yes, as long as you have 10 years of ADI membership cumulatively, you are eligible to apply.

 

Can I apply for Tier 4 without publications?

A: No. A minimum of 2 peer-reviewed publications is mandatory.

 

Can I sit the Tier 4 exam before meeting the membership requirement?

A: No. You must meet the 10-year membership requirement before sitting the exam.

 

 

Tier 4: Examination 

What is the format of the Tier 4 examination?

A: The Tier 4 examination consists of three parts:

  • Written multiple-choice exam
  • OSCE (Objective Structured Clinical Examination)
  • Critical appraisal
 

When will the first Tier 4 examination take place and what will the syllabus cover?

A: This is currently to be confirmed.

 

I have a disability or additional learning need. Can reasonable adjustments be made for the online Tier 4 examination?

A: Yes. The ADI is committed to ensuring that the Fellowship examination is accessible to all candidates. If you have a disability, medical condition, neurodiverse condition, or other circumstance that may affect your ability to undertake the online examination, please contact [email protected] as early as possible.

Reasonable adjustments will be considered on an individual basis and may include additional examination time, supervised rest breaks, or other appropriate arrangements for the online assessment. Supporting evidence may be requested where appropriate, and all requests will be treated confidentially.
