Treating the severely atrophic maxilla has long been considered the remit of maxillofacial surgeons with extensive bone/iliac crest grafting, requiring hospital admission, lengthy recovery and extended treatment times. This has understandably led to poor patient uptake of these complex procedures.

The zygomatic implant offers a treatment alternative to major grafting, which can be carried out in the practice setting under local anaesthetic and conscious sedation. The high stability achieved with this implant allows for immediate loading. The zygomatic implant can be combined with either anterior axial implants or for severely atrophic, with two zygomatic implants each side (Quad zygoma).

  • Classification of atrophic maxilla
  • Treatment alternatives for the atrophic maxilla
  • Major grafting procedures with clinical cases, including complications
  • The zygoma implant including indications, planning, use of stereolithographic models, technique and recent advances
  • Use of the zygoma implant as a “rescue implant” for failing implants

Biography

Dr Guy McLellan is doubly qualified in dentistry and medicine and a Fellow of the Royal College of Surgeons of England. He has over 10 years’ hospital experience including training in ENT and surgery and has an Honorary Consultant post in Oral Implantology at Charing Cross Hospital, running the dental implant clinic.

His experience in implants dates back to 1994 and includes extra-oral implants to retain facial cosmetics/reconstruction following facial cancer surgery as well as intra-oral cancer rehabilitation and facial deformities such as cleft lip and palate. A member of the Cranio-Facial team at Chelsea and Westminster Hospital. In private practice he limits his practice to surgical implantology and augmentation procedures including complex intra-oral and extra-oral grafting.

He is a mentor for Nobel Biocare and is actively involved in Implant education. He is a member of the Association of Dental Implantology and the European Association of Osseointegration.
